Peninsula Dental Associates provides quality dental care for many patients in Foster City, CA, and throughout the Peninsula. We’re always happy to take on new patients of all ages. You can get started today by checking out the information and forms below.

First Visit

We are thrilled to welcome you to your first visit at Peninsula Dental Associates. During this appointment, we will start by discussing your medical history to provide you with personalized care. Our team will then conduct an initial exam, and if necessary, take x-rays to assess your oral health. Finally, we will discuss treatment options tailored to your specific needs. We look forward to helping you achieve a healthy and beautiful smile!

Please bring with you to the first appointment:

Valid ID
Insurance card (if applicable)
X-rays from previous provider (if applicable)
List of medications you are currently taking

Please note: All patients under the age of 18 must be accompanied by a parent or guardian

Insurance

We accept all PPO insurance plans. Payment for services is due at the time of service and our friendly and helpful staff will submit insurance claims on your behalf. We are also happy to help you with any required pre-authorizations.
